Financial Aid Forms

In order to view the PDF forms online, you must have Adobe Acrobat Reader. Please visit www.adobe.com in order to download the software.

If you fill out the PDF version, please mail them to: Financial Aid Office, Lake-Sumter State College, 9501 U.S. Hwy 441, Leesburg, FL 34788 or drop them off at the Financial Aid Office at the Leesburg, South Lake, or Sumter Campus

Dynamic Forms

You can complete, sign and submit your financial aid forms to us electronically. To complete the dynamic forms: 

• Login to your LOIS account through www.lssc.edu
• Select Financial Aid
• My eligibility
• Student Requirements
• Select aid year and click submit 

Please note: Parents of dependent students must create their own account to complete the process.

• First time users must use the “Create Account” button before logging in create a Dynamic forms user account.
• Returning users should “Sign In” using their most recent login criteria.

Please allow 5 business days for the processing of Dynamic Forms.

*Faxed documents will not be accepted and your file will be considered incomplete until the original forms are received.
