Financial Aid Forms

If you fill out the PDF version, please mail them to: Office of Financial Assistance, Lake-Sumter Community College, 9501 U.S. Hwy 441, Leesburg, FL 34788 or drop them off at the Office of Financial Assistance at the Leesburg, South Lake, or Sumter Campus

New Dynamic Forms

You now have the option to complete, sign and submit your financial aid forms to us electronically. To complete the dynamic forms, select the link to the form you wish to complete.
Please note: Parents of dependent students must create their own account to complete the process.

• First time users must use the “Create Account” button before logging in create a Dynamic forms user account.
• Returning users should “Sign In” using their most recent login criteria.

Please allow 5 business days for the processing of Dynamic Forms and during peak times processing can take one (1) week to 10 days.

*Faxed documents will not be accepted and your file will be considered incomplete until the original forms are received.
